New patient

What to expect at your first visit

During your first visit, our objective is to get to know you better, to become familiar with your past dental experiences and to establish a treatment plan that will best meet your current desires. When you arrive for your first visit, please be prepared to complete all insurance and health information forms.

You will receive a link for you to fill out with your New Patient forms. The link will remain active for 3-days, if you need a new link sent to you, let us know. We ask for you to fill out forms promptly in order to provide the following information:

  • You will fill out a release of records, this allows us to retrieve x-rays from your previous provider.
  • Have your insurance card available while filling out forms as you will be providing insurance details.
  • If forms have not been filled out 48-hours prior to your appointment we will reach out and reschedule as needed.
  • Note, for cancellations within 24-hours of the scheduled appointment there is a fee of $75.

More Insurance Information

Here, we prioritize your convenience and peace of mind. We gladly accept most major insurance plans, and our dedicated staff is committed to billing your insurance plan.

The best way for you to know what your insurance will cover when seeing a provider is to reach out to your insurance company directly—either through your online patient portal or by calling their member services line. We recommend asking them if you will be able to take advantage of in-network benefits at our office. If Buffalo Dental Group is out-of-network for your specific plan, you can ask them what your insurance will contribute toward your services.

Please note: Buffalo Dental Group is not an enrolled provider for Medical Assistance programs

Regarding Medicare Plans: We are at capacity with Blue Cross Blue Shield Medicare and are not taking new patients with this plan. We are out-of-network with Aetna Allina Medicare and United Healthcare Medicare

Out-of-Network: Some insurance carriers we are out-of-network with;
Aetna (Network particpation is pending, contact your member line to confirm)
Aetna Allina Medicare
United Healthcare Medicare
Cigna Andvantage and DHMO Plans
GEHA
Guardian
If Buffalo Dental Group does not participate In-Network with your insurance, know that you can still come here. Although you will not be able to take full advantage of what your insurance would contribute when seeing an in-network provider, we will still gladly bill your insurance company so they can contribute their portion toward your care.
