What to expect at your first visit

During your first visit, our objective is to get to know you better, to become familiar with your past dental experiences and to establish a treatment plan that will best meet your current desires. When you arrive for your first visit, please be prepared to complete all insurance and health information forms.

Also, please assist us by providing the following information:

  • Any panoramic or full set series of X-ray taken within 3 to 5 years.
  • If you have dental insurance, please bring your insurance card. This will enable us to give you an estimate of your costs.

We gladly accept most major insurance plans, and our dedicated staff is committed to working closely with you to navigate and comprehend your specific insurance coverage. We understand that each patient’s needs are unique, and we work to ensure that you receive the highest quality care and maximize your benefits.

We are not providers for any Medical Assistant Programs.
We are at capacity with Blue Cross Blue Shield Medicare and are not taking new patients with this plan.
We are not able to treat patients with the Allina Aetna Medicare Grand Plan. All other Aetna Allina Medicare Plans, we can bill insurance and patients will receive out-of-network contribution.
We are out-of-network with GEHA, Guardian, Cigna Advantage PPO and United Healthcare Medicare; we can bill insurance and patients will receive out-of-network contribution.
